Should any officer pension contributions already pensioned be excluded ?

Officer post pension contributions should not be included on a Type 1 form. Only Practitioner posts should be included.

Are 'officer' payments included in the total pensionable income, even though they do not go on the Type 1 form?

No, officer income is not included for total pensionable pay and should not be recorded on the Type 1 form.
