PCSE will administer all Information Disclosure requests for deceased individuals from
- Coroners
- GPs
- PHSO
- Police
Any other organisations will be directed to contact the last registered practice of the patient, unless this is closed - in which case PCSE will process.
If you are directed to the last practice then please note that any records held by PCSE would be requested by the last practice and reviewed for disclosure alongside any digital records they hold when they process a request for access to a deceased patient records.
