As of the 1st of July 2024, PCSE will be responsible for administrating the process of notices of withdrawal from a pharmaceutical list. The completed forms are to be sent to PCSE who will then forward to the relevant integrated care board. 

Pharmacy and dispensing appliance contractors who wish to withdraw premises from a pharmaceutical list are required to give a period of notice: 

  • pharmacies with 40 core opening hours – at least three months 
  • pharmacies that are, or have been subject to, the 100 hours condition – at least six months.
  • pharmacies with more than 40 core opening hours where a direction is in place – at least three months. 
  • dispensing appliance contractors – at least three months. 

However, regulation 67 of the NHS (Pharmaceutical and Local Pharmaceutical Services) Regulations 2013, as amended, states that where it is impracticable for the contractor to give the required notice period, they must notify NHS England (since 1 April 2023, the integrated care board in whose area the premises are located) as soon as it is practicable to do so. In addition, the integrated care board can agree to a shorter notice period.

The Chapter 38 Annex 1 - Closure of premises notification should be completed by contractors where they wish to withdraw premises from a pharmaceutical list.

Please note, this form is not to be used by contractors who provide services under a local pharmaceutical services (LPS) contract or by dispensing doctors who wish to withdraw premises from a dispensing doctor list. 

Any requests to withdraw a premises from a pharmaceutical list that have been submitted previously, for example, before 1 July 2023, will still be dealt with by your local ICB.
