Change your practice bank details

If you need to update your practice bank account details for GP payments, you must submit a completed Declaration of Banking Details form.

We use a standardised form to ensure requests are processed quickly and securely.

Before you submit

Before sending your request, please ensure:

  • you have completed the standard Declaration of Banking Details form
  • all mandatory fields are completed in full
  • the form is submitted by a user with an approved PCSE Online role
  • the form is signed by all GP partners at the practice

Requests that do not meet these requirements will be rejected.

Who can submit a request

Bank mandate requests must be submitted by a practice contact with one of the following PCSE Online roles:

  • GP Principal
  • Non-GP Partner
  • Statements
  • Practice User Management

Requests submitted by users without one of these roles will not be processed. 

How to submit your request

  1. Download and complete the Declaration of Banking Details form
  2. Ensure all required signatures are included
  3. Submit your request via the Contact Us page: 
     
    • Category: GP Payments
    • Payment query type: Change of practice bank details
  4. Attach your completed form

Important requirements

  • All GP partners must sign the form
  • The form must be fully completed
  • The submission must be made by an approved role

Any request that does not meet these requirements will be rejected and returned for resubmission. 

What happens if your request is incomplete

Your request will be rejected if:

  • it is not submitted using the standard form
  • not all GP partner signatures are provided
  • it is submitted by a user without the correct role
  • mandatory fields are missing or incomplete

This may delay processing as you will need to resubmit a corrected request.

Transition period

A transition period is in place to support these changes.

  • Requests already in progress can be submitted until 31 July 2026.
  • From 1 August 2026, all requests must use the updated form and meet the revised requirements

Any requests submitted after this date that do not comply will be rejected.
