How do I request an 8-day removal?

To request an 8-day removal, the most efficient method is to submit an online form

The preferred notification method for the most efficient processing is via the Request for Removal From Practice List Form.

Can I request an 8 day removal for patients under the age of 18 ?

Yes, patients under the age of 18 can be removed on an 8-day removal. Howeverconfirmation that the Safeguarding Lead has been consulted must be provided. Failure to provide this will result in the request being rejected.

When can I request an 8-day removal?

8-day removal requests can be initiated by the GP due to a result of a breakdown in relationship between the GP and the patient, for example due to repeated non-attendance of appointments (DNA). See more information on our 8 day removal page.

Why has my request for removal been rejected?

PCSE will reject a removal request when it does not meet the criteria outlined in the relevant section of NHS England Primary Medical Care Policy and Guidance Manual (PGM). See more information on our 8 day removal page.
