My details and the NHS

Overview

We have recently written to some patients to confirm if the home address the NHS holds for them is up to date. GPs need to have up to date addresses for their patients to ensure you receive important information about your health.

We have collated information to help you understand why you have received a letter about your registration with your GP Practice and what you need to do.

If you need the information in another language, please use the link below to access the letter and form in your language.

About the letter and what it means

PCSE carry out data quality checks on patient lists on behalf of NHS England. To do this, we may send a letter to registered patients to ensure that the details held on the system we use are accurate and up to date.

It is vital that data held by both PCSE and the patient’s registered GP practice is correct, as the data is used by various screening programs to determine their eligibility of patients, as well as sending invites and result letters. The letter you received asks you to confirm the details currently held for you.

Why data quality checks are important

If your GP does not have an up to date record of your home address, then they may not be able to contact you with important information about your health.

The list of patients registered at each GP practice is used by the NHS to contact patients to offer appointments, invite them to screening programmes and to provide test results.

This includes screening for bowel, breast and cervical cancer, and appointments for vaccinations against infectious diseases such as flu, measles, mumps and rubella.

What you need to do now

Please follow the instructions on the enclosed form to confirm the NHS hold the correct details for you. There are instructions explaining how to respond included on the letter. Your options include text message, online form or postal form.

If you believe you have been wrongly removed from your GP Practice, please let your practice know in the first instance. Your practice will contact us to understand the reason for patient removal, and we'll provide advice on what action they need to take.
