How to report a confusion to PCSE
Follow the steps below to see how a practice reports a confusion between patients.
1. Identify the Confusion
A confusion occurs when two or more patients are mistakenly linked to the same NHS number, corrupting demographic and clinical records.
2. Submit the Online Form
The practice must complete the PCSE online form available on the PCSE Contact Us page.
- Select 'New Duplicates and Confusions Enquiry'.
- Include as much detail as possible:
- Patient names
- NHS numbers
- Dates of birth
- Previous addresses
- GP practice codes
Important: this information should be confirmed with the patient directly.
Patient data present on your practice system may contain information relating to both patients’ involved in the confusion. PCSE cannot contact patients directly in this situation.
3. Submit the query.
You will be provided with a case number in the following format CAS-1234567-123456
