What happens next?
Once the form has been submitted, PCSE will investigate:
- If further information is required PCSE may contact your practice or any other practice that is involved to request additional information.
- Once enough information has been provided PSCE will log a duplicate request with the National Back Office (NBO).
Please note NBOs Investigation may take up to 10 weeks.
- When the NBO have provided a resolution, PCSE will contact you to confirm the correct NHS number to use.

- Practices cannot contact PCSE directly by email for updates; If an update is required please visit the PCSE Contact Us page and select “Existing Duplicates and Confusions Enquiry”. You will need your case reference number.
- Once a case is logged to National Back Office (NBO) it may take up to 10 weeks to resolve, depending on how much information is received on the original form, and also National Back Offices current timescales.
- Practices should not reply to automated PCSE emails, as these are not monitored. To respond to an email, or provide further information relating to a case, please visit the PCSE Contact Us page and select “Existing Duplicates and Confusions Enquiry”. You will need your case reference number.
- Obtain as much information as possible from the patient before submitting the online form to prevent delays. This includes any previous addresses, names, and place of birth. If insufficient information is received on the form, PCSE may need to request you contact your patient for further information.
