How to report a duplicate to PCSE
Follow the steps below to see how a practice reports a duplicate between patients.
1. Identify the Duplicate
A duplicate occurs when a single patient is allocated more than one NHS number, resulting in multiple records for the same individual.
2. Submit the Online Form
The practice must complete the PCSE online form available on the PCSE Contact Us page.
- Select “New Duplicates and Confusions Enquiry”.
- Include as much detail as possible:
- Patient name/Change of name/Previous surname(s)
- NHS numbers
- Dates of birth
- Previous addresses
- Previous GP practice codes
Important:
Important: This information should be confirmed with the patient directly.
PCSE cannot contact patients in this situation.
3. Submit the Query. You will be provided with a case number in the following format CAS-1234567-123456
