Solely Locum work
If you are a Locum GP who only completes locum work, you do not need to complete either form.
Partner and locum work
If you complete Partner and locum, or Partner and solo and locum work, you will need to complete a Type 1 Annual Certificate.
Salaried and locum work
If you complete Salaried and locum, or Salaried and solo and locum work, or solo and locum work, you need to complete a Type 2 Self Assessment.
