How to inform PCSE of an Immediate Removal
Online Form
The preferred notification method for the most efficient processing is via the Request for Removal From Practice List Form.
If you are unable to use the online form, you can also contact PCSE via:
Please complete the Removal Request Paper Form and email to pcse.patientremovals@nhs.net

The incident must have been reported to the Police for the immediate removal to be processed. Secondary or local commissioner approval will not be necessary.
If the police have not been informed or the incident is being reported 7 days after the reporting period commissioner approval is required. The commissioner will need to fill in the ICB NHSE Approval Form. Once completed the form will need to be submitted to PCSE alongside the request.
The updated policy includes a provision for the Special Allocation Scheme (SAS) provider to determine whether the removal request is appropriate.
What happens when a patient is removed from a GP Practice
Patients who are removed from their GP Practice due to an incident at the practice requiring police action are allocated to a specific GP Practice who manage Special Allocation Scheme (SAS) Patients.
The patient is notified by letter that they have been placed on the scheme and are given details of the practice and contact information should they wish to arrange appointments. They are also advised in the letter that they must not contact or attend the practice who requested their removal. Patients will be required to attend appointments only at the designated SAS practice.
SAS Practices will provide the same services as non-SAS Practices. However, their choice of GP and entitlement to home visits may be restricted.
The practice who has requested the removal will receive an acknowledgement that the removal has been completed
How long does a patient stay on the scheme
An annual assessment is carried out by clinicians. PCSE will be informed only if the patient can be discharged from the scheme. A decision may be made earlier than 12 months on a case-by-case basis. PCSE have no input into this process.
PCSE will advise the patient that they have been removed from the scheme and can seek registration with a practice of their choice.
